Trainee advanced clinical practitioner

The post holder will have a level of extensive experience in the care of the older person. Acting within their professional boundaries and scope of practice they will be responsible for a level of assessing presenting patients, history taking, clinical assessment, assisting diagnosis, medication review and evaluation of care. The post holder will have access to Consultant Geriatric and ANP Support. The post holder will demonstrate safe, clinical decision-making care, critical thinking and work collaboratively with the multidisciplinary team to meet the needs of the patient. The post holder will support the delivery of policy and procedures, providing service co-ordination and leadership where required
