Full Job Description

  • Work collaborativelywith mental health professionals and service users tomanage and improve physical health outcomes.

  • Provide education and supportto service users on healthy lifestyle choices, including nutrition, physical activity, and preventative healthcare.

  • Liaise with other healthcare providers, such as GPs and community nurses, to ensure coordinated care for service users., To oversee the physical health checks for our service users, ensuring that routine tests are carried out annually or more often if necessary.

  • Educating and supporting our service users to take care of their physical health needs as well as mental health.

  • Supporting our service users to engage in physical health interventions, including healthy eating, sleep, exercise, and smoking cessation.

    You are a qualified healthcare professional with experience working with individuals with mental health conditions.

  • You are passionate about promoting physical well-being and integrated healthcare.

  • You are a skilled communicator and team player with the ability to build rapport with service users and other professionals.
