Full Job Description

We are seeking to appoint a Systems Change Coordinator: Healthy Communities Together - Leeds (37 hours).

Aims

  • To develop, maintain and influence systems partnerships, building them into strong relationships which benefit marginalised communities' engagement in health systems.

  • To develop and deliver the Leeds HCT project activities in conjunction with the Solidarity Network and NHS and Local Authority partners keeping the needs of marginalised communities central at all times.

  • To manage multiple workstreams in a complex project with different partners.

  • To manage participation and communications for the project.

  • To produce engaging reports and products.

  • To reduce health inequalities experienced by marginalised communities in Leeds.

  • To involve, train, supervise and support volunteers/community leaders and/or experts by lived experience from marginalised communities - using a community development approach.

  • To develop and deliver a programme of facilitated spaces for Leeds' health care systems professionals, in conjunction with Solidarity Network, Local authority, NHS partners and Marginalised community members.

  • To liaise with and work with other service providers to develop and deliver joint pieces of experimental work to benefit the HCT project outcomes. (e.g. GP services, same days response providers, Gender Identity Clinic team).

  • To ensure effective communications for the project including Twitter, Facebook, press, mailing lists and events.

  • To take the lead on producing reports for the Healthy Communities Together project; for the Kings Fund, the National Lottery, The Solidarity Network and other partners.

    In Leeds the Solidarity Network, with our NHS and local authority partners, have been awarded funds to deliver phase two of Healthy Communities Together (HCT). HCT is an exciting funding strand from the National Lottery Community Fund in collaboration with the Kings Fund; supporting systems change in health care services through VCS, NHS and local authority working together. We aim to move from transactional to mutual partnerships to impact and benefit marginalised communities of Leeds and improve poor health outcomes. The marginalised communities this project aims to ensure are involved in their health care are Gypsies and Travellers, Sex Workers, Asylum Seekers and Refugees and Trans+ communities. The Solidarity Network are: Leeds GATE, BASIS Yorkshire, LASSN and Yorkshire MESMAC.

    Salary: NJC (scale points 23-25) starting at £32076 per annum for 37 hours per week. Contract is fixed one year term to 31st June 2025. Salary is reviewed annually. There is a 5% workplace pension (on completion of probationary period). This post is subject to an Enhanced DBS with barred lists check.
