We are expanding our team to meet the evolving needs of our healthcare system and are looking for a skilled Pharmacy Technician to join our dynamic team. This strategic investment aims to improve medication management, support chronic disease management, and enhance overall efficiency within our practice., Job Summary: As a Pharmacy Technician at Birchwood Medical Practice, you will play a critical role in improving medication management, supporting chronic disease management, and handling medication queries efficiently. You will work closely with our Clinical Pharmacist and other healthcare professionals to ensure the accuracy and safety of medication dispensing, conduct medication reviews, and provide patient education to promote adherence and reduce the risk of adverse events.,

  • Improved Medication Management:

  • Assist in medication reconciliation for new patients, ensuring adherence to local and ICB formulary guidance.

  • Identify and resolve medication discrepancies to improve patient outcomes.

  • Address patient concerns and provide clear instructions to promote medication adherence and reduce the risk of non-compliance.

  • Support for Chronic Disease Management:

  • Conduct medication reviews for patients with chronic conditions such as diabetes, hypertension, and asthma.

  • Identify potential drug interactions, side effects, or gaps in therapy and collaborate with healthcare providers to optimize treatment plans.

  • Efficient Medication Queries Handling:

  • Serve as the initial point of contact for medication-related queries from patients, pharmacies, and other healthcare professionals.

  • Address queries promptly and accurately, reducing the workload of GPs and ensuring timely resolution of medication issues.

    Registered Pharmacy Technician with GPhC.

  • Minimum of 1 years of experience in a primary care setting is preferred.

  • Strong knowledge of medication management and reconciliation processes.

  • Excellent communication and organisational skills.

  • Ability to work independently and as part of a multidisciplinary team., Pharmacy: 1 year (preferred)
