Full Job Description

The Mental Health Living Well Service are recruiting additional staff to complement their current offer. There are 6 services across Rochdale, Heywood and Middleton providing assessment, triage, short term interventions and support to individuals struggling with their mental health.
You will be working alongside a team of varying professions including:
+ Senior mental health practitioners (PCNs)
+ Associate Psychological Practitioners (APPS)
+ Consultant Psychiatrist
+ Adult Care
+ Substance Misuse Services
+ Wellbeing Coordinators
+ Peer support workers/Lived experience practitioners
+ Rough Sleeper/ dual diagnosis services
+ Social Prescribers
+ Criminal Justice
The role involves undertaking comprehensive assessments and providing expert advice, education and support to individuals. You will provide guidance in risk management and help signpost and refer on to relevant services where required. You will be expected to provide short term interventions and work in collaboration with community based services to aid recovery. You will have the support of the MDT and the consultant psychiatrist to formulate robust management plans.
The service covers 7 days a week, inc bank holidays however we perceive the working week will mainly be Monday- Friday with some flexibility to offer support to individuals outside of these hours. The service is committed to providing a supportive working environment therefore flexi working applications are considered., + The post will involve being based within one of the 6 sectors in HMR, you will be part of a wider MDT who come together to support individuals in their times of need, you will be working independently and alongside colleagues as part of the network.
+ The post will involve triaging referrals within the huddles and assessing the appropriateness for mental health support, and signposting to other services if not appropriate.
+ This post will be responsible for the assessment and formulation of presenting mental health problems experienced by individuals at a secondary care level.
+ You will be responsible for delivering brief psychological based interventions for people experiencing common mental health problems.
+ You will be providing short term support to individuals to support with strengths based support, help with strategies for anxiety management and living with low mood, making community connections, trauma informed care, practical support amongst a wide range of other interventions.
+ Implement recovery pathways
+ This post involves liaison with other roles and services within the community.

The service receives routine and urgent referrals into mental health services, the team use a huddle approach where each referral is discussed and together a individualised plan is made that would best meet their needs. We aim to ensure there is a true multi agency approach to the support we offer, we strive to provide excellent care using a strengths based approach and encourage engaging with community based/ voluntary based offers in local areas.
